Release of Liability
I, hereby acknowledge that my participation in the Athlete Performance Program or Athlete Insights sessions, conducted by Fitamentalist Pty Ltd, trading as Athlete Engineers, involves certain inherent risks. In consideration for being permitted to participate in the program, I agree to release and discharge Fitamentalist Pty Ltd, trading as Athlete Engineers, its officers, employees, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me during the program.
I understand and acknowledge the risks involved in the activities conducted as part of the Athlete Performance Program or Athlete Insights sessions, and I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury, including death, that may be sustained by me as a result of participating in the program.
I further agree to indemnify and hold harmless Fitamentalist Pty Ltd, trading as Athlete Engineers, its officers, employees, and agents from any loss, liability, damage, or costs, including attorney's fees, that they may incur due to my participation in the program.

Indemnity
I agree to defend, indemnify, and hold harmless Fitamentalist Pty Ltd, trading as Athlete Engineers, its officers, employees, and agents from and against any and all claims, suits, demands, or actions of any kind, including but not limited to third-party claims, arising from or related to my participation in the Athlete Performance Program or Athlete Insights sessions.
I understand and acknowledge that this agreement shall be binding upon me, my heirs, and personal representatives, and shall inure to the benefit of Fitamentalist Pty Ltd, trading as Athlete Engineers, its successors, and assigns.


UNDER 18 YEARS PARENT / GUARDIAN CONSENT
Where Participant is under 18 years of age:
I being the parent/guardian of the person named in the acknowledgement and release hereby acknowledge and agree that:
• I have read the whole of this document and understand it.
• I consent to the person named in this acknowledgement and release participating in the activity and
• I am aware of the risks, dangers and obligations set out above in the acknowledgement and release.
In Consideration of the person named in the Acknowledgement and Release being accepted to participate in the activity.
I agree to release and indemnify Fitamentalist Pty Ltd trading as Athlete Engineers in the same manner and to the same effect and extent as if I were the person first named in this Acknowledgement and Release and the person participating in the activity.

Photo and Video Release
I hereby grant Fitamentalist Pty Ltd, trading as Athlete Engineers the irrevocable and unrestricted right to use and publish photographs, videos, and/or audio recordings of me that may be taken during the Athlete Performance Program, or Athlete Insights sessions for any lawful purpose, including but not limited to publicity, advertising, and web content.
I understand that these materials may be used in various publications, public affairs releases, online media, or other related endeavours. I also understand that I will not be compensated for the use of these materials.
I release and discharge Fitamentalist Pty Ltd, trading as Athlete Engineers from any and all claims and demands arising out of or in connection with the use of the photographs, videos, and/or audio recordings, including any and all claims for libel or invasion of privacy.

By signing below, I acknowledge that I have read and understand this release of liability, photo and video release, and indemnity form, and I voluntarily agree to its terms.
